Elimination Doubles Tournament Entry Form
1. 230 maximum team average.
2. Teams may be made up of two men, two women, or mixed
3. PROOF of average verification must be brought to the tournament.
4. Tournament average used is your VERIFIED AVERAGE AS OF MARCH 1ST, 2014.
5. Entry Fee is $80.00 per team; MONEY ORDERS PREFERRED, payable to
Lakeside Lanes. Cash or Personal Checks if dropping off entry form

*Directions to Lakeside Lanes - Take 93 North or South to Exit 7, Route 101 East,
sign will read, Portsmouth Seacoast. From Exit 7 take Exit 1, Route 28 By-pass, Auburn- Hooksett
exit. At the light, take a right, follow to the rotary, bearing right. 500 ft on left side
is Lakeside. From Maine...Route 95 south to Route 101 West. stay on 101 West until
you come to exit 1, again sign will read. Auburn-Hooksett Route 28 by-pass. take left, to
rotary, bear right, 500 ft on left side is Lakeside Lanes.
